Welcome to the Penn-Trafford Health Services Page!

Each of our eight schools is overseen by a Certified School Nurse (CSN). Schools may also be staffed by a Registered Nurse (RN) in addition to the CSN. If your child has special health care needs, requires medications or health treatments during the school day, and/or has a chronic health condition, please contact your school nurse to discuss.
 
 
 
All Students (K-12) need the following immunizations for attendance:
  • 4 doses of tetanus, diphtheria, and acellular pertussis (usually given as DTP or DTaP or DT or Td; 1 dose given on or after the 4th birthday)
  • 4 doses of polio (4th dose on or after 4th birthday and at least 6 months after the previous dose given)
  • 2 doses of measles, mumps, and rubella (usually given as MMR)
  • 3 doses of hepatitis B
  • 2 doses of varicella (chickenpox) or evidence of immunity
 
Students attending 7th-12th grade need the following additional immunizations for attendance:
  • 1 dose of tetanus, diphtheria, acellular pertussis (Tdap)
  • 2 doses of meningococcal conjugate vaccine (MCV)
    • 1st dose is given 11-15 years of age
    • 2nd dose is required at age 16 or entry into 12th grade
    • If the dose was given at 16 years of age or older, only 1 dose is required
 
Exemptions to the school laws for immunizations are:
  • medical reasons;
  • religious beliefs; and
  • philosophical/strong moral or ethical conviction
 
 
If your child is exempt from immunizations, he or she may be removed from school during an outbreak.

  • Height, Weight, and Vision - Grades K - 12
  • Hearing - Grades K - 3, 7 and 11

The Pennsylvania School Health Code mandates that all immunization records and physicals are required of new or transfer students to Penn-Trafford School District. They will be reviewed prior to your child's first day of school.
 
Please bring a copy of your child's immunization record and physical to the Nurse's office for review or your child will not be permitted to begin school. There will be no exceptions to this mandate. In addition, Student Emergency Contact Information and Student Health Forms must be completed in the Parent Portal of PowerSchool.
