Mask Exemption Form

On August 31, 2021, the Acting Secretary of Health issued an Order requiring all individuals to wear a face covering in all school entities effective Tuesday, September 7, 2021 unless they have a medical or mental health condition or disability, documented in accordance with Section 504 of the Rehabilitation Act or IDEA, that precludes the wearing of a face covering in school. 

If your child qualifies for a mask exemption, you must complete the below form, including the physician section, and submit it to Nurse Popovich via email at [email protected].  Your child should continue to wear a mask at school until you are notified of approval.
